2. Immunsuppressive Therapie
CAVE: Zielspiegel der Immunsuppressiva sind abhängig von der
verwendeten Bestimmungsmethode!
Induktionstherapie
Anti-Thymozyten-Immunglobulin (ATG)
®
Thymoglobuline (Fa. Genzyme)
1,5 – 2 mg/ kg KG intravenös über 4 – 8 h� Start ca� 2 h nach Ankunft auf ICU
sofern keine Blutungsneigung auftritt, Therapiedauer 3 – 10 Tage�
Die Gesamtdosis sollte zwischen 4,5 – 7,5 mg / kg KG liegen�
Weitere Dosierung wird nach Thrombozytenzahl errechnet�
• > 100000: 1,5 – 2 mg/ kg KG
• > 50000: 1 – 1,5 mg/ kg KG
• > 30000: 1 mg/ kg KG + Thrombokonzentrat oder Pausierung
• < 30000: Pause + Thrombokonzentrat
Nebenwirkungen: Thrombopenie, Leukopenie, Infusionsreaktionen
(Fieber, Schüttelfrost, Übelkeit, Tachykardie, Anaphylaxie)
Erstgabe mit Kortison, Antihistaminika, Metamizole (z�B� Novalgin) koppeln�

ATG-Fresenius (Fa. Neovii):
2 – 5 mg/ kg KG intravenös über 4 h� Therapiedauer 5 – 10 Tage�
Alternative
Basiliximab (Simulect , Fa. Novartis):
®
2 x 20 mg innerhalb von 5 Tagen post HTX (z�B� Dosis 1: Tag 0, Dosis 2: Tag 5)
®
Alemtuzumab (Campath , Fa Sanofi):
30 mg intravenös während HTX, vor Reperfusion�
